How trauma contributes to addiction — the neuroscience of trauma and substance use, adverse childhood experiences (ACEs), and why trauma-informed care is essential for effective treatment.
The relationship between trauma and addiction is one of the most well-established findings in addiction research. Studies consistently show that people with addiction are significantly more likely to have experienced trauma — particularly childhood trauma — than the general population. The ACE (Adverse Childhood Experiences) study, one of the largest investigations of childhood trauma and adult health ever conducted, found that people with four or more adverse childhood experiences were seven times more likely to develop alcohol use disorder and ten times more likely to inject drugs than people with no adverse childhood experiences.
This relationship is not coincidental. Trauma and addiction are connected through multiple neurobiological, psychological, and social mechanisms that help explain why people who have experienced trauma are so much more vulnerable to addiction — and why effective treatment for addiction so often requires addressing trauma. Understanding this connection is essential for anyone seeking to understand addiction, whether in themselves or in someone they love.
Trauma is the psychological and physiological response to an event or series of events that overwhelms a person's capacity to cope. Traumatic events are those that involve actual or threatened death, serious injury, or sexual violence — either experienced directly, witnessed, or learned about through a close relationship. But trauma is not defined solely by the event itself; it is defined by the impact of the event on the person who experiences it. Two people can experience the same event and have very different responses, depending on their prior experiences, their support systems, and their neurobiological makeup.
Trauma can be acute (a single event, such as an accident or assault), chronic (repeated events over time, such as ongoing abuse or domestic violence), or complex (multiple, varied traumatic events, often beginning in childhood and involving interpersonal harm). Complex trauma — which is particularly common in people with addiction — has the most profound and lasting effects on brain development, emotional regulation, and interpersonal functioning.
Trauma — particularly early childhood trauma — produces lasting changes in the brain's stress response systems, the same systems that are dysregulated by addiction. Traumatic experiences activate the amygdala (the brain's fear center) and the hypothalamic-pituitary-adrenal (HPA) axis (the body's primary stress response system), producing a state of hyperarousal that can persist long after the traumatic event has passed.
In people with PTSD, this hyperarousal becomes chronic: the stress response system remains activated even in the absence of threat, producing the anxiety, hypervigilance, emotional dysregulation, and intrusive memories that characterize the condition. The prefrontal cortex — which normally modulates the stress response and helps regulate emotions — is impaired by chronic stress, reducing the person's capacity for emotional regulation and impulse control.
Substances — particularly alcohol, opioids, and benzodiazepines — provide temporary relief from this chronic hyperarousal. Alcohol reduces anxiety and promotes relaxation by enhancing GABA activity and reducing glutamate activity. Opioids reduce both physical and emotional pain by activating the brain's natural pain-relief system. Benzodiazepines reduce anxiety by enhancing GABA activity. For people with trauma histories, these substances can feel like the first relief they have ever experienced from the chronic distress of their trauma — which is one of the primary reasons that people with trauma histories are so vulnerable to addiction.
The ACE study, conducted by Kaiser Permanente and the Centers for Disease Control and Prevention in the 1990s, assessed the relationship between childhood adversity and adult health outcomes in more than 17,000 adults. The study identified ten categories of adverse childhood experiences: physical abuse, emotional abuse, sexual abuse, physical neglect, emotional neglect, domestic violence, household substance abuse, household mental illness, parental separation or divorce, and incarceration of a household member.
The study found a strong dose-response relationship between ACE scores and the risk of addiction: the more adverse childhood experiences a person had, the greater their risk of developing a substance use disorder. People with an ACE score of 4 or more were 7 times more likely to develop alcohol use disorder, 10 times more likely to inject drugs, and 12 times more likely to attempt suicide than people with an ACE score of 0. These findings held even after controlling for other risk factors, suggesting that childhood adversity is an independent risk factor for addiction.
The ACE study has been replicated in multiple populations and countries, and its findings have been enormously influential in shifting the understanding of addiction from a moral failing to a health condition with identifiable developmental origins. The question "What's wrong with you?" has been replaced, in trauma-informed care, with the question "What happened to you?" — a shift that is both more accurate and more compassionate.
The self-medication hypothesis — the idea that people use substances to relieve the symptoms of untreated mental health conditions, including trauma-related conditions — is particularly relevant to the trauma-addiction connection. People with PTSD, depression, anxiety, and other trauma-related conditions often discover that substances provide temporary relief from their symptoms, and they use substances increasingly to manage the emotional pain that their trauma produces.
This self-medication pattern is not a sign of weakness or poor judgment — it is a rational response to unbearable pain in the absence of better alternatives. Understanding this helps reduce the shame that people with trauma histories often feel about their addiction, and it points toward the importance of addressing the underlying trauma in treatment. If the addiction is a symptom of untreated trauma, treating only the addiction without addressing the trauma is unlikely to produce lasting recovery.
Post-traumatic stress disorder (PTSD) and substance use disorder co-occur at very high rates. Studies suggest that approximately 50 percent of people seeking treatment for substance use disorder have PTSD, and approximately 30–50 percent of people with PTSD have a co-occurring substance use disorder. This co-occurrence is not coincidental — PTSD and substance use disorder share neurobiological mechanisms, and each condition worsens the other.
PTSD increases the risk of substance use disorder by producing the chronic distress that drives self-medication. Substance use disorder worsens PTSD by impairing the brain's capacity for emotional regulation, disrupting sleep (which is essential for trauma processing), and creating additional traumatic experiences (accidents, violence, legal problems) that compound the original trauma. This bidirectional relationship means that treating one condition without addressing the other is unlikely to be effective.
Trauma-informed care is an approach to treatment that recognizes the prevalence and impact of trauma, integrates knowledge about trauma into all aspects of treatment, and seeks to avoid re-traumatizing people in the treatment process. It is based on four key principles: safety (creating physical and emotional safety in the treatment environment), trustworthiness (being transparent and consistent), choice (giving people control over their treatment), and collaboration (working with people rather than doing things to them).
Trauma-informed care does not require every treatment provider to be a trauma specialist. It requires all providers to understand the prevalence of trauma, to ask about trauma history as part of routine assessment, to create treatment environments that feel safe and supportive, and to refer to trauma specialists when needed. It also requires providers to recognize that behaviors that might appear to be resistance or non-compliance — such as missing appointments, difficulty engaging in treatment, or emotional dysregulation — may be trauma responses rather than character flaws.
Research consistently shows that trauma-informed approaches to addiction treatment produce better outcomes than approaches that ignore trauma. Integrated treatment programs that address both trauma and addiction simultaneously — such as Seeking Safety, which is one of the most widely studied integrated treatments — have been shown to reduce both PTSD symptoms and substance use more effectively than sequential treatment (treating one condition first, then the other).
Recovery from co-occurring trauma and addiction is possible, but it typically requires more comprehensive and sustained treatment than recovery from addiction alone. Effective treatment addresses both conditions simultaneously, using evidence-based approaches for both trauma (such as Prolonged Exposure, Cognitive Processing Therapy, or EMDR) and addiction (such as cognitive-behavioral therapy, medication-assisted treatment, and peer support).
The recovery journey for people with trauma histories is often longer and more complex than for people without trauma. Setbacks are common, and they should be understood as part of the recovery process rather than as evidence of failure. The person who has experienced significant trauma and who is working to recover from both trauma and addiction is doing some of the hardest work a human being can do — and they deserve compassion, patience, and comprehensive support.
Many people with severe trauma histories have achieved stable, long-term recovery and have gone on to live full, meaningful lives. Their recovery is a testament to the resilience of the human spirit and to the power of appropriate treatment and support. Recovery is not just possible — it is the expected outcome when people receive the care they need.
