A comprehensive overview of alcohol use disorder — how it develops, its stages and symptoms, the health consequences of chronic alcohol use, and the evidence-based pathways to recovery.
Alcohol use disorder (AUD) is the clinical term for what is commonly called alcoholism or alcohol addiction. It is defined by the DSM-5 as a problematic pattern of alcohol use leading to clinically significant impairment or distress, characterized by at least two of eleven specific symptoms occurring within a twelve-month period. AUD exists on a spectrum from mild (2–3 symptoms) to moderate (4–5 symptoms) to severe (6 or more symptoms).
AUD is one of the most prevalent substance use disorders in the world. In the United States, approximately 14.5 million adults — about 5.8 percent of the adult population — have AUD in any given year. Globally, alcohol use is responsible for approximately 3 million deaths annually, making it one of the leading preventable causes of death worldwide. Despite its prevalence and its devastating consequences, AUD remains dramatically undertreated: fewer than 10 percent of people with AUD receive any form of treatment in a given year.
AUD typically develops gradually, over months to years of escalating alcohol use. The progression is not inevitable — many people drink heavily for periods of time without developing AUD — but for those with genetic vulnerability, certain environmental exposures, or co-occurring mental health conditions, the progression can be rapid.
The development of AUD typically follows a recognizable pattern:
Early stage: Drinking increases in frequency and quantity. The person begins to use alcohol to manage stress, anxiety, or social discomfort. Tolerance develops — more alcohol is needed to achieve the same effect. The person may begin to plan their activities around drinking opportunities and may feel uncomfortable in social situations where alcohol is not available. At this stage, the person may not recognize that their drinking has become problematic, and others may not notice either.
Middle stage: Drinking becomes more central to the person's life. They may begin to drink alone, to drink in the morning, or to drink to manage withdrawal symptoms. Relationships, work, and health begin to be affected. The person may make repeated attempts to cut down or stop, with limited success. They may begin to hide their drinking from family and friends, and to feel shame and guilt about their use. Blackouts — periods of amnesia during heavy drinking — may become more frequent.
Late stage: Drinking is the organizing principle of the person's life. Physical dependence is established — stopping drinking produces severe withdrawal symptoms. Health consequences are significant. The person may have lost their job, their family, and their home. They may be drinking continuously to avoid withdrawal. At this stage, the person is typically unable to stop drinking without medical assistance, and the consequences of continued drinking are life-threatening.
The DSM-5 criteria for AUD include eleven symptoms, and a diagnosis requires the presence of at least two within a twelve-month period. The severity of AUD is determined by the number of symptoms present:
Chronic heavy alcohol use affects virtually every organ system in the body. The health consequences of AUD are extensive and can be life-threatening:
Liver disease: The liver is the primary organ responsible for metabolizing alcohol, and it bears the brunt of chronic heavy use. Alcohol-related liver disease progresses through three stages: fatty liver (steatosis), which is reversible with abstinence; alcoholic hepatitis, which can be severe and life-threatening; and cirrhosis, which is irreversible scarring of the liver that affects approximately 10–15 percent of people with severe AUD. Cirrhosis can lead to liver failure, portal hypertension, and hepatocellular carcinoma.
Cardiovascular disease: Heavy alcohol use is associated with increased risk of cardiomyopathy (weakening of the heart muscle), arrhythmias including atrial fibrillation, hypertension, and stroke. The relationship between alcohol and cardiovascular disease is complex — moderate alcohol use has been associated with reduced cardiovascular risk in some studies, but heavy use is clearly harmful, and the risks outweigh any potential benefits for people with AUD.
Neurological damage: Chronic heavy alcohol use causes direct neurotoxic damage to the brain, producing cognitive impairment, memory problems, and in severe cases, Wernicke-Korsakoff syndrome — a devastating neurological condition caused by thiamine (vitamin B1) deficiency that produces severe memory impairment, confusion, and in its chronic form (Korsakoff syndrome), an inability to form new memories. Peripheral neuropathy — damage to the nerves of the hands and feet — is also common in people with severe AUD.
Cancer: Alcohol use is a known carcinogen, classified as a Group 1 carcinogen by the International Agency for Research on Cancer. It is associated with increased risk of cancers of the mouth, throat, esophagus, liver, colon, rectum, and breast. The risk increases with the amount consumed and is not limited to heavy drinkers — even moderate alcohol use is associated with some increased cancer risk.
Mental health: AUD is strongly associated with depression, anxiety, and other mental health conditions. The relationship is bidirectional: mental health conditions increase the risk of AUD, and AUD worsens mental health conditions. Approximately 50 percent of people with AUD have a co-occurring mental health condition, and effective treatment must address both.
Immune system: Chronic alcohol use suppresses the immune system, increasing susceptibility to infections including pneumonia and tuberculosis. It also impairs wound healing and increases the risk of post-surgical complications.
Alcohol withdrawal is one of the most medically serious withdrawal syndromes of any substance. Unlike opioid withdrawal — which is intensely uncomfortable but rarely life-threatening — alcohol withdrawal can be fatal. The severity of alcohol withdrawal depends on the duration and quantity of alcohol use, and it ranges from mild symptoms (anxiety, tremor, sweating, insomnia) to severe symptoms (seizures, hallucinations, delirium tremens).
The most severe form of alcohol withdrawal, delirium tremens (DTs), typically develops 48–72 hours after the last drink and involves seizures, hallucinations, severe agitation, and cardiovascular instability. Without treatment, DTs carries a mortality rate of 5–15 percent. With appropriate medical treatment — typically involving benzodiazepines to prevent seizures and manage symptoms — the mortality rate drops to less than 1 percent.
Anyone who has been drinking heavily and regularly should seek medical supervision before stopping. Medically supervised detoxification is the standard of care for alcohol withdrawal. Attempting to stop drinking "cold turkey" without medical supervision can be dangerous and potentially fatal.
Recovery from AUD is possible, and it takes many forms. The most effective approaches combine multiple elements tailored to the individual's needs, circumstances, and preferences:
The goal of treatment is not necessarily complete abstinence for everyone — some people achieve significant improvement through reduced drinking rather than complete abstinence, and harm reduction approaches have an important role to play. However, for people with severe AUD, abstinence is typically the safest and most sustainable goal, and the evidence for abstinence-based recovery is strong.
Alcoholics Anonymous, founded in 1935, remains one of the most widely available and widely used resources for people with AUD. AA's 12-step program provides a structured framework for recovery that addresses the psychological, social, and spiritual dimensions of addiction. Research on AA's effectiveness has consistently found that regular AA attendance is associated with better long-term sobriety outcomes, and that the mechanisms of AA's effectiveness include social support, accountability, spiritual practice, and the development of coping skills.
AA is not the only pathway to recovery from AUD, and it is not the right fit for everyone. But for the millions of people who have found sobriety through AA, it has been a life-saving resource. The combination of AA with evidence-based medical treatment — including MAT where appropriate — represents the most comprehensive approach to AUD recovery currently available.
